ThinkForm is providing professional services and documentation for a complete renovation of the Primary Care Department on the 2nd floor of Building 205 to accommodate a Patient Aligned Care Team (PACT) Unit at the Togus VA Medical Center. The project includes renovation of up to approximately 18,159 square feet of existing space. Primary Care Unit (PACT) represents a significant change in the VA’s approach to the environment of care to a “patient-centered, team based” model of care. Design objectives for PACT space planning and design are based on the guiding principles of providing Patient-Centered Care, Coordination of Care, and Access to Care.
The Patient-Centered Care design will create a calm, healing environment, and support operation and flow to reduce waiting and wasteful or redundant activities. The Team Based Care design will colocate staff to support and enhance direct staff communication, create team-based office space, enhance work flow and establish zones to improve privacy. The Access to Care design will standardize room design, provide space for shared medical appointment group care, and support a modular approach to space planning. ThinkForm Architects will continue its collaboration with all critical stakeholders during the VA’s review of the conceptual plan options with the intent of incorporating comments into the work as the design progresses into the following phase.
